Our promise is to provide quality health care to all, regardless of a patient’s insurance coverage or ability to pay. Our goal is to make it easier for you to make appointments, get answers to your health care questions, and take charge of your personal health.
Discount Policy
We offer sliding fee discounts to low-income patients with out insurance or inadequate insurance. Please ask to see our Benefits Coordinator for determining your sliding fee discount. Sliding fee discounts are determined based on your family size and income.
Patients will not be denied services based on their inability to pay.

Our sliding fee discounts are based on the Federal Poverty Level (FPL) guidelines as updated annually by the U.S. Department of Health and Human Services. To determine your sliding discount eligibility, please review the guidelines by clicking on the link below.

Preventative Services: Exams, x-rays, cleanings, fluoride treatments, education, sealants.
Other Core Services*: Fillings, simple extractions, pulp capping & pulpotomy, recementing, denture adjustments & repairs, removal of space maintainers.
Additional Services*: Periodontal treatment, space maintainers, dentures, denture rebasing & relining, surgical extractions, other surgical procedures.
Elective Services: Root canals, crowns, bridges, bleaching.
* Some Procedures have an additional Lab/Material Fee – you may request a Treatment Plan explaining all costs prior to any non-preventative dental services.

We are committed to providing integrated, affordable & quality health care to you and your family. We are proud to accept a wide array of insurances, Medicaid and Medicare plans, and offer self-pay options for your convenience and accessibility.
NOTE: Before each visit, please call your health plan to verify that the provider you’re seeing participates with your plan. Not all providers participate with every plan.
